The part numbers we have on the box and switch are KG32B, K300*CD0002E, KNKB23075/005 1106.  Though we didn't see a correlation in their catalog we believe the closest match is
KG32B T203/01 E.
This switch has 4 hole mounting and is lockable (use a padlock through the open holes in switch cover, can only be locked with the contacts in the open position to interrupt the flow of current) for safety.  The switch is rated for 32A.  This is a 3 Pole switch with maintained contacts, at 480V on 3 phase it can handle a 10 HP motor or on single phase a 5 HP motor (see pictures).   It is a rotary switch 90 degrees.
